does the premium end automatically after 12 months or do i have to cancel it ?

Log out and log back in while your Steam profile isn't 'private'. Your Steam account must be older than 1 year to get verified status.
You can set it back to private afterwards.
(This is to protect the system from resource abuse from mass-created new Steam accounts.)
